Introduction
The Three Affiliated Tribes of the Fort Berthold Reservation – North East Segment in conjunction with Elbowoods Memorial Health Center is preparing for the construction of a new ambulatory healthcare facility which will serve the medical needs of the community of Parshall as well as the surrounding communities. Elbowoods Memorial Health Center is seeking proposals for professional Architectural and Engineering Services for the Parshall Community. Companies responding to this request must submit their proposal in accordance with the enclosed requirements. (1) hard copies and (4) electronic PDF copies of the completed response must be received no later than July 11th, 2025 at 11:59 PM CST.
Requested Services
Elbowoods Memorial Health Center is requesting architectural/engineering services on an as needed basis including, but not limited to assistance with planning and preparation for a small ambulatory healthcare facility, project design, and services to administer construction projects to the extent required by the construction manager. Elbowoods would expect the architect to provide approve submittals, issue change orders, certify payments and administer construction projects. Various general needs projects may be needed throughout the year and the chosen architect/engineer would be involved with those as well. The construction contract will be administered pursuant to the Indian Self-Determination and Education Assistance Act, Public Law (P.L.) 93-638, and applicable sections of 25 Code of Federal Regulations Part 900, Subpart J (Construction Contracts). These regulations will be used to provide Federal assistance for this project.
Evaluation and Selection Procedure
Proposals will be evaluated by a selection committee. The successful proposal will be based upon the submitted information. Proposals that meet satisfactory levels of criteria will be considered. Proposals that do not meet these requirements will not be evaluated.

Proposal Requirements
The proposed project will be designed to meet the needs of the medical staff and of the public at large, but is not limited to the following:

Fully functional medical facility to support the following services
· Primary Care
· Physical Therapy
· Pharmacy
· Dental
· Optometry
· Laboratory
· Radiology
· Public Health Nursing / Community Health
· Behavioral Health
· Patient Registration
· Administration

On-site roads, parking lots, curb and gutter, lighting and striping as required.

On-site Infrastructure:
· Electrical
· Storm water
· Sanitary Sewer
· Domestic Water and Fire Line
· Communications including Security

Representative concept design drawings for infrastructure, clinic, parking lot and garage.

Cost estimate for design document drawings for the infrastructure, clinic, parking lot and garage.

Cost estimate for construction including general conditions and administrative fees for complete vertical construction of the infrastructure, clinic, parking lot and garage.

Cost estimate for site improvements including utility installation tie-in from curb to gutter. The base price should include the use of geothermal heat pumps with a diesel back-up generator.

Coordination with The Three Affiliated Tribes of the Fort Berthold Reservation – North East Segment and Elbowoods Memorial Health Center in the development of off-site infrastructure.

Company must be willing and able to obtain a Performance Bond.

Must have a current MHA TERO permit or have a statement of intent to obtain an MHA TERO permit.

Firm Introduction: 2 Page Max

Name of company submitting proposal along with address, phone number, and contact person.

ND State Business License or General Contractor License.

Copy of the company’s Limited Liability Insurance and OHSA 30 card.

Team Qualifications: 5 Pages Max excluding resumes

Provide an organizational chart of the full team required to deliver the project.

Provide resumes of Key personnel who will be actively engaged in the project.

Provide availability of staff who will be involved in the project.

Provide the number of years the team members have worked for the company as a team.

Project Experience: 20 Page Max

Provide information on relevant projects experience. Include the project name, cost, and schedules for up to three comparable projects pages with images. Identify which team members proposed for this project, also worked on the three project examples shared.

Provide the volume of 7-12 work you have done in the last 5 years in dollars.

Provide the number of Tribes you have done architectural projects for in the past 5 years.

Please share what you believe will be the biggest risks in this project and how you will approach them.
