Sports Physical (7th-12th grades)
A sports physical helps to determine if it is safe for the student to participate in a sport. These exams are typically REQUIRED before an athlete can start a new sport or a new competitive season. If able, please print off and complete the North Dakota High School Activities Association (NDHSAA) form PRIOR to your scheduled appointment and bring with you. If you can not print and complete prior to the appointment, please arrive 20 minutes prior to complete the form at the clinic.
**Parent or legal guardian must be present with the student for the appointment. Bring ID and proof of insurance if applicable.**
